National Health Insurance Scheme

STEP BY STEP REGISTRATION PROCESS FOR THE NATIONAL HEALTH INSURANCE SCHEME.

The National Health Insurance Management Authority is established pursuant to section 4 of the National Health Insurance Act No. 2 of 2018. The main objective of the Authority is to implement and operate the National Health Insurance Scheme, manage the National Health Insurance Fund, accredit health insurance healthcare providers, develop a comprehensive benefits package to be accessed by members, register & issue membership cards to members and facilitate member access to quality health service Thus, for purpose of implementation of the Act, the National Health Insurance (General) Regulations No. 63 of 2019 was issued. The Act provides for the manner of registration of a citizen or established resident into the scheme and the requirements a citizen or established resident should satisfy before being registered as a member of the scheme. The Act provides for three ways in which a citizen or established resident can be registered as a member of the scheme with the Authority, these being:

(a) Employer Registration.
(b) Pension Manager Registration.
(c) Self Registration.

The third schedule of the regulations sets out the contribution rates as follows:

Employer Registration


Step 1

Employers should submit their details under cover letter. Details of the employer should include:
  • Registered Trade Name
  • PACRA Registration Number
  • TPIN
  • Contact Person
  • Contact Person Cell Phone Number
  • Contact Person Email Address

Attached to the cover letter is a schedule of employees, which should include the following information:
  • Employee Name (i.e. Surname, forename & first name as they appear on NRC)
  • Is applicant a citizen or established resident? (Note established resident is as defined in the Immigration and Deportation Act of 2010).
  • State nationality of the applicant
  • Title
  • Sex
  • NRC Number
  • Date of Birth
  • Passport Number for non-Zambians
  • Marital Status
  • Employment Permit Number (where applicable)
  • Physical Address
  • Basic Pay
  • 1% Employee Contribution
  • 1% Employer Contribution

Step 2

The total Employee and Employer Contribution payment can be deposited into the following bank account:

Step 3

The Cover Letter, Employee Schedule and Proof of Payment of Contributions should be sent to:

Step 1

log onto enhima.nhima.co.zm and register employer. Fill in details of Employer in the portal. Details of employer include:
  • Registered Trade Name
  • PACRA Registration Number
  • TPIN
  • Contact Person
  • Contact Person Cell Phone Number
  • Contact Person Email Address

The registration of the Employer on the NHIMA portal will then be approved and confirmed.

Step 2

Once confirmed, the Employer will be required to upload a schedule of Employees with the following information:

  • Employee Name (i.e. Surname, forename & first name as they appear on NRC)
  • Is applicant a citizen or established resident? (Note established resident is as defined in the Immigration and Deportation Act of 2010).
  • State nationality of the applicant
  • Title
  • Sex
  • NRC Number
  • Date of Birth
  • Passport Number for non-Zambians
  • Marital Status
  • Work Permit Number (where applicable)
  • Physical Address
  • Basic Pay
  • 1% Employee Contribution
  • 1% Employer Contribution

The file format for the upload will be CSV.

Step 3

Once the CVS is uploaded, the Employer will be required to upload the proof of payment either as a file upload or make a payment through the online payment portal.:

Step 4

Submit the application for review and approval of the application and await the confirmation of registration of each Employee.

COMPLETION OF FORM I

The Administrator should distribute Form I (attached or downloaded from NHIMA Forms ) to each Pensioner on the schedule for completion with the attendant documents as indicated on the form. Form I is attached and may be printed and completed by each pensioner. This may be done after registration or before to manage the process.

Each Administrator will have 60 days in which to ensure that the Form I is submitted to the Authority supporting the registration done above.

Pension Manager Registration


Step 1

It must be noted however, that pensioners above 65 years are exempt from contributing to the scheme. Pensioner below the age of 65 years are required to contribute at 0% of their income. Therefore, the Pension Scheme Manager or Administrator must follow the following steps:

The Pension Fund Administrator or Manager should submit details of the Pension Fund under cover letter. The following details should be contained in the cover letter:
  • Registered Pension Scheme Name,
  • TPIN
  • Contact Person
  • Contact Person Cell Phone Number
  • Contact Person Email Address

Attached to the cover letter is a schedule of pensioners, which should include the following information:
  • Pensioner’s Full Name. (i.e. Surname, forename & first name as they appear on NRC)

Self Registration


Step 1

A self-employed citizen or established resident should will be required to complete Form I (attached) or downloaded from the website (www.nhima.co.zm).

Complete Form I. Note that Part A is mandatory and must be fully completed. All the required documentation should be attached to the form on submission as indicated.

Step 2

The applicant will be required to complete part B 2 of Form and indicate their average monthly income and the sector that they are employed in

Step 3

The applicant will then be required to pay an amount of 1% of the declared monthly income as indicated in the registration form. The payment should be made into the following bank account

Step 4

The proof of payment should be attached to the application form.

Step 5

Once the form has been duly completed and signed, the forms and all attendant attachments including the proof of payment are submitted to the Authority for review to the following:
